If you would like to register your child(ren) for grades K through 5, please print the following registration forms and and bring them to the school with your child's birth certificate (for Kindergarten students; in addition to the forms below, please follow the link above).

If your child will require bus transportation, please complete the following form and return it with the registration forms.
